07/14/20 W-9 Form Required with The Borough's CARES Act Relief Fund Grant Application

Working on a Borough CARES grant application tonight?

Be sure to include a current IRS W-9 form with the application. The W-9 form also known as a "Request for Taxpayer Identification Number and Certification" asks for basic tax identification number. A TIN can be also be a social security number or employer identification number.

TIP: Before signing the W-9, be sure the TIN matches the tax information listed on your application. Keep a copy of the W9 with your application documents. You can download a fillable W-9 from the IRS official site at https://www.irs.gov/pub/irs-pdf/fw9.pdf.
